Star Dental respects your privacy. This Website Privacy Policy explains the information we may collect when you visit mystardental.com, how we use it, and the choices available to you. The separate HIPAA Notice of Privacy Practices below explains how Star Dental may use and disclose protected health information in connection with dental care.
Information We Collect Through This Website
- Information you choose to provide. This may include your name, telephone number, email address, appointment preferences, and the contents of a message submitted through a contact or appointment-request form.
- Technical and usage information. Our website and service providers may automatically receive information such as your IP address, browser and device type, referring page, pages viewed, approximate geographic area, and the date and time of a visit.
- Call and interaction information. We may measure clicks on telephone numbers, directions, email links, and appointment-request buttons so we can understand whether the website is useful and whether our advertising is effective. If a call is recorded, we will provide any notice or obtain any consent required by applicable law.
How We Use Website Information
We may use website information to respond to requests, help schedule appointments, provide requested information, operate and secure the website, improve our services, measure website and advertising performance, prevent fraud or misuse, and comply with legal obligations.

Ordinary website forms and email are not intended for emergencies or for sending detailed medical or dental information. If you have an urgent dental concern, call 303-222-1414. We retain website information only as long as reasonably necessary for the purposes described above, to maintain appropriate records, and to meet legal obligations. HIPAA Notice of Privacy Practices

This notice describes how medical and dental information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Who This Notice Covers
This Notice describes the privacy practices of Star Dental and the members of its workforce who provide or support your care. In this Notice, “we,” “our,” and “us” mean Star Dental, and “you” and “your” mean the patient.
Your Rights
When it comes to your health information, you have certain rights. Contact our Privacy Officer using the information at the end of this Notice to exercise these rights.
Get an electronic or paper copy of your records
- You may ask to see or receive an electronic or paper copy of your dental, medical, and billing records and other health information we maintain about you.
- We will provide a copy or summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee as permitted by law.
Ask us to correct your records
- You may ask us to correct health information that you believe is incorrect or incomplete.
- We may deny the request in some circumstances, but we will explain the reason in writing, generally within 60 days.
Request confidential communications
You may ask us to contact you in a particular way, such as only at a certain telephone number, or to send mail to a different address. We will agree to reasonable requests.
Ask us to limit what we use or share
- You may ask us not to use or share certain information for treatment, payment, or health care operations. We are not always required to agree, and we may deny a request if it could affect your care.
- If you pay for a service or health care item out of pocket in full, you may ask us not to share information about that item or service with your health plan for payment or health care operations. We will agree unless a law requires us to share it.
Get a list of certain disclosures
- You may ask for an accounting of certain disclosures of your health information made during the six years before your request, including who received the information and why.
- The accounting will not include certain disclosures, such as those made for treatment, payment, or health care operations. One accounting in a 12-month period is free; we may charge a reasonable, cost-based fee for additional requests during that period.
Get a copy of this Notice
You may ask for a paper copy of this Notice at any time, even if you agreed to receive it electronically. We will provide one promptly. The current Notice is also available on our website and in our office.
Choose someone to act for you
If a person has legal authority to act for you, such as a medical power of attorney or legal guardian, that person may exercise your rights and make choices about your health information. We will verify the person’s authority before acting.
File a complaint
- You may complain to Star Dental if you believe your privacy rights have been violated by contacting our Privacy Officer below.
- You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by writing to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting hhs.gov/hipaa/filing-a-complaint.
- We will not retaliate against you for filing a complaint.
Your Choices
For certain health information, you may tell us your choices about what we share. Tell us your preference and we will follow your instructions when the law gives you that choice.
- You may tell us whether to share information with family members, close friends, or others involved in your care or payment for your care.
- You may tell us whether to share information in a disaster-relief situation.
If you are unable to tell us your preference, such as when you are unconscious, we may share information if we believe it is in your best interest. We may also share information when necessary to lessen a serious and imminent threat to health or safety.
We will obtain your written permission before using or sharing health information for marketing, selling your information, or most uses of psychotherapy notes. Star Dental does not sell protected health information. If we contact you for fundraising, you may tell us not to contact you again.
How We Typically Use or Share Health Information
Treat you
We may use your health information and share it with dentists, physicians, laboratories, pharmacies, specialists, and other professionals involved in your care. For example, we may share an X-ray or treatment history with a specialist who is evaluating you.
Run our practice
We may use and share your health information to operate our practice, improve care, train our workforce, conduct quality review, contact you when necessary, and manage our services.
Bill for services
We may use and share your health information to bill and obtain payment from Medicaid, dental benefit plans, health plans, and other payers. For example, we may send information about treatment to your insurer so it can determine coverage or pay a claim.
Work with business associates
We may share health information with vendors that help us perform services such as billing, records storage, software support, secure communications, or legal and accounting work. When required, these business associates must sign agreements requiring them to appropriately protect the information.
Other Uses and Disclosures Permitted or Required by Law
We may use or share health information in other ways permitted or required by law, and we must meet applicable legal conditions before doing so. These situations may include:
- Public health and safety activities, such as preventing disease, reporting adverse reactions or product problems, reporting suspected abuse or neglect, or preventing a serious threat to health or safety.
- Health research when legal requirements are satisfied.
- Compliance with federal or state law, including disclosures to the U.S. Department of Health and Human Services to demonstrate compliance with federal privacy law.
- Organ and tissue donation requests.
- Disclosures to a coroner, medical examiner, or funeral director when a person dies.
- Workers’ compensation claims, health oversight activities, and certain law-enforcement or government requests.
- Responses to a court or administrative order, subpoena, lawsuit, or other legal proceeding as permitted by law.
Substance use disorder records. To the extent we maintain substance use disorder patient records protected by 42 CFR Part 2, we will not use or disclose those records in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a court order accompanied by a subpoena. If fundraising communications would use Part 2 information, we will give clear advance notice and a choice about whether to receive them.
Uses and disclosures not described in this Notice will be made only with your written authorization unless otherwise permitted or required by law. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
Our Responsibilities
- We are required by law to maintain the privacy and security of your protected health information.
- We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information.
- We must follow the duties and privacy practices described in the current version of this Notice and provide you with a copy.
- We will not use or share your information other than as described in this Notice unless you authorize us in writing. You may change your mind at any time by telling us in writing.
Changes to This HIPAA Notice
We may change the terms of this Notice, and the changes may apply to all health information we maintain, including information created or received before the change. The current Notice will be available upon request, in our office, and on our website.
