How to Verify Health First Colorado Dental Eligibility

You do not necessarily need a physical Medicaid card to begin an eligibility check. Health First Colorado members can view status through PEAK or the mobile app, and a participating dental office can often verify active eligibility using identifying information such as the member’s full name and date of birth.

Information that helps the office find your record

  • Legal first and last name exactly as used for Health First Colorado
  • Date of birth
  • Current address and phone number
  • Health First Colorado member ID, if available
  • Photo ID for the appointment

If you do not know the member ID, do not postpone calling. Star Dental can often look up eligibility with your name and date of birth and will tell you if another detail is needed. Benefit verification confirms the information available at that time; a specific procedure may still require clinical documentation or prior authorization.

Three ways to check Health First Colorado status

  1. Ask the dental office. An enrolled provider can check current eligibility before treatment. This is useful when you need an appointment and want the office to confirm participation and coverage.
  2. Use Colorado PEAK. Sign in at Colorado PEAK to view or manage benefits. PEAK also shows application information and communications.
  3. Use the Health First Colorado app or Member Contact Center. The app can display an electronic member card and benefit status. The Member Contact Center is 1-800-221-3943.

What if the system says coverage is inactive?

Inactive status can result from a renewal, missing information, a household change or another eligibility issue. The dental office cannot reactivate coverage. Check PEAK communications and contact the appropriate county eligibility office or Health First Colorado. If you recently submitted an application or renewal, ask when the update is expected and whether more documents are required.

What to bring to your first dental appointment

Bring if available: photo ID, Medicaid or DentaQuest card, a medication list and relevant dental records. Arrive 10–15 minutes early because new-patient forms are completed at the office.

Tell us: the reason for your visit, how long symptoms have been present, allergies, medical conditions and whether another dentist has recommended treatment.
